This Week’s Focus: NDEWS Early Warning in Action — Insights from the NDEWS Early Warning Network including Sentinel Site Directors Report

The NDEWS Weekly Briefing focuses on our Sentinel Site Directors (SSD) report, which highlights trends involving nitazenes. This question was prompted by recent NDEWS data that highlighted a decline in nonfatal nitazene-related overdoses along with a growing negative sentiment for nitazenes among online Reddit communities. 

This briefing also highlights a recent study led by NDEWS affiliate and Columbia University NIDA T32 Postdoc Nicole Fitzgerald, along with NDEWS Co-Directors Linda Cottler and Joseph Palamar. The study used NDEWS Rapid Street Reporting survey data to examine the adverse outcomes associated with the use of new psychoactive substance use (NPS).

Additionally, we feature our NDEWS Medicolegal Death Investigators (MDI) – Network Winter 2026 Quarterly Report. For this report, we collected information from medical examiners, coroners and/or affiliated staff such as toxicologists, forensic professionals and others on nitazenes, carfentanil, and notable trends from 2025, in addition to other local trends.

Drug poisoning deaths appeared to again decline in 2025, down more than 30% from the 2023 peak in King County.

Drug-related deaths were stable in 2025 after a reduction in 2024. Fentanyl continues to drive mortality. Emerging data suggest a significant proportion of fentanyl overdose deaths involve intermittent, first-time, or unintentional use of fentanyl.

Harm Reduction Coalition of San Diego County shared that some participants have mentioned "pigeon's blood", a red heroin that costs more and is stronger. According to participants, this type of heroin has not been available for 10 years, but has now returned. No evidence available from drug checking at this time.

As of this year, more interviews with our cohort members have revealed a much higher concern about fentanyl in the their supply. There has also been an increase in reports from cohort members in both knowing people who have recently experienced an overdose and knowing those who have died from an overdose. Paired with wastewater surveillance data this suggests that there is minimally more fentanyl present in the region and that our cohort members are aware and concerned by this.

In St. Louis County, drug deaths have continued to decline from a high of 397 in 2021 to 217 in 2024 and then 260 in                                2025. A review of results from the 267 drug samples submitted to St. Louis County's drug checking lab (Project EAGLE FANG) in 2025 show that the submitted combinations have started to change dramatically. A “typical” sample in December 2024 would contain cocaine, Benadryl, fentanyl, lidocaine, methamphetamine, xylazine in white powder (submitted as crack) and a submitted sample of fentanyl (gel cap) contained 6-monoacetylmorphine cyclobenzaprine, DMT ,MDMA, methamphetamine, and olanzapine. In January 2025, BTMPS and heroin were identified in drug mixtures. In July, a higher percentage of fentanyl reported, still in a mixture of drugs, ranging from 22-44%. In September 2025, the the percentage of fentanyl dropped, to low levels, with occasional xylazine, BTMPS, medetomidine, and carfentanil identified. All samples contained fentanyl or heroin, methamphetamine, cocaine, ketamine, or MDMA, and occasional trace amounts of xylazine along with psychoactive and inert substances such as Tylenol. The samples were submitted as bags, gel caps, pills, powder, pipes. Fentanyl continues to be the primary drug used for treatment admission followed by cocaine, methamphetamine, and then heroin.

Based on the preliminary 2025 toxicology data from Houston Forensic Science Center, as fentanyl cases decrease, hydrocodone shows signs of increasing.

Toxicology laboratory directors continue to report significant decreases in the prevalence of new psychoactive substances (NPS) in decedents including novel benzodiazepines, cathinones, fentanyl analogs, and nitazenes in decedents. Cocaine, fentanyl, and methamphetamine continue to be the predominate substances identified. Laboratories are closely monitoring the shift of nitazenes to -orphine derivatives. Another substance of concern is 7-hydroxymitragynine (7-OH), a pharmacologically-active metabolite of mitragynine (Kratom), with mu-opioid receptor affinity significantly greater than morphine. 7-OH is a Schedule I controlled substance in the state of Florida. NPS and other substances identified in toxicology casework includes alpha-PiHPP (isoPV8), bromazolam, carfentanil, desalkylgidazepam, etizolam, ketamine, methylenedioxymethamphetamine, MDMB-4en-PINACA, ortho-methyl fentanyl, and pyrazolam. Adulterants identified in the quarterly crime laboratory drug report include BTMPS, medetomidine, and xylazine.

Recently, a patient came to the ED seeking treatment for opioid use disorder withdrawal symptoms reported to be from 7-OH. Withdrawal symptoms improved after 2 mg of buprenorphine was administered based on Clinical Opiate Withdrawal Scale (COWS) score.

Medicolegal Death Investigators (MDI) – Network Quarterly Report, Winter 2026: Anecdotes from the field (39 responses across 24 states)

The tables below provide more insight on shifts in the caseloads from the MDI network over the past 6 months. Of 39 respondents, 18 observed a decrease in suspected overdose deaths. Regarding polysubstance use cases, 16 respondents reported that they saw about the same rate of cases (high), and 17 said that they saw about the same rate of substances co-detected (high) in their casework.

In this quarterly survey, we collected information on the observed shifts in overdoses and their characteristics. For the Winter 2025 report, we received a total of 39 responses from 24 states. Of those, 30 people (~77%) completed extended responses about nitazenes and carfentanil detected in decedents and post-mortem toxicology, or medical examiner/coroner reports over the past 6 months. Responses also highlighted notable local trends in casework observed in 2025 compared to the previous year(s) and other trends of note. To read the full report, click here.

Self-reported adverse effects associated with new psychoactive substance use in a sample of adults from 20 US cities

A recently published study in Drug & Alcohol Review led by NDEWS affiliate and Columbia University NIDA T32 Postdoc Nicole Fitzgerald, along with NDEWS Co-Directors Linda Cottler and Joseph Palamar, examined NDEWS Rapid Street Reporting (RSR) survey data to examine the adverse outcomes associated with the use of new psychoactive substance (NPS). The authors reported that data from 6,039 adults from 23 data collection visits to 20 unique cities between January 2022 and November 2023. The results indicated that among 259 individuals who reported past-year NPS use, 27% experienced an adverse effect from the use of at least one NPS and those also reporting past-year opioid use had over two times the prevalence of reporting an adverse effect related to NPS use.. Availability of nitazenes via cryptomarkets and surface web shops: An observational study

A recently published observational study in Addiction by Man et al. examines the availability of nitazines for purchase on international cryptomarkets and surface web shops. Data were collected from 34 cryptomarkets on a bi‑monthly basis from October 2021 to September 2024, and an additional 10 surface‑web retail sites were identified in April 2024 through a search‑engine query examining the availability of nitazenes for purchase in Australia. The authors note that these market insights show the importance of ongoing digital trace monitoring. Read the full study here.
